Consent for WhatsApp video call interview

The form below consists of 13 statements which you must answer ‘yes’ to in order to be part of the study. Once you have agreed to all the statements, a short contact form will appear for you to fill in.

1. I have read and understood the Participation Information Sheet [Version 3, dated 02/03/2021] for the above project.

2. I have had the opportunity to consider the information, ask questions and have had these answered satisfactorily (where applicable).

3. I understand that participation in the study involves being interviewed by a researcher about my experiences of depression during my partner’s pregnancy or during the 2 years after the birth of my baby.

4. I understand that my interview will be audio recorded with digital recording technology which will be retained until the PhD is awarded and subsequently deleted.
Video component will not be recorded.

5. I agree to my words being used as quotes in publications/presentations and that these will be anonymised so I won’t be identifiable.

6. I understand that my participation is voluntary and that I am free to leave the study at any time.

7. Consent for the researcher to use your interview data can be withdrawn up to 7 days after your interview. At this point interview data will be anonymised and it will no longer be possible to know who the data belongs to. Personal data will be kept separately and destroyed upon completion of the project or upon withdrawal from the study.

8. I understand that data collected during the study, may be reviewed by individuals from Edge Hill University or from regulatory authorities for audit purposes where it is relevant to my taking part in this research. I give permission for these individuals to have access to my anonymised data.

9. I understand that this consent form will be stored as per the detail given in the Participant Information Sheet dated [Version 3, dated 02/03/2021].

10. I understand that the information collected from me may be used to support other research in the future, and I agree for my anonymous research data to be shared with other researchers.

11. I understand that should I disclose any information about illegal activity or risk of harm to self or others, the researcher is duty bound to report it to the relevant authorities.

12. I understand that if it becomes apparent that I am in a mental health crisis, the researcher will contact my next of kin and/or the appropriate service.

13. I hereby grant Edge Hill University authority to process my personal and research data for the purpose of my participation in the above-named research study. I also understand that my personal contact details will not be passed to anyone outside of the research team at Edge Hill University. If I wish to withdraw my consent to process my personal data in the above-named study, at any time, I will do so by contacting the research team using the details on the information sheet.
